Telemonitorización en domicilio para pacientes Pluripatológicos con EPOC e Insuficiencia Cardiaca.

Los pacientes de nuestro área del Aljarafe, se benefician de un programa de medición diaria en su domicilio de constantes vitales.
Este programa, que se ha empezado a desarrollar hace pocos meses, mide en domicilio automáticamente, la tensión arterial, la frecuencia cardiaca, la cantidad de oxigeno en la sangre, la capacidad de los pulmones y el peso ( este último como expresión de la retención de liquido). De esta forma, se facilita la identificación precoz de las descompensaciones de enfermedades cardiacas y respiratorias graves en estadio avanzado, y así se puede modificar el tratamiento para poder controlarlas.
A fecha de hoy un total de 30 pacientes, se están midiendo diariamente estas variables , con los sistemas habituales de domicilio. Tras la medición, los resultados se envian automaticamente a una central de telemonitorizacion. Esta central clasifica esta información y a traves de un enfermero contacta con el paciente para comprobar clinicamente su situación. Si existe descompensación, se contacta rápidamente con Salud Responde, indicandose el nivel de gravedad. Desde Salud Responde, se gestiona aviso directamente con su medico de familia o si es grave con el 061. Si se necesita, el paciente es trasladado al Hospital.
En estos dias, se ha reunido el equipo de evaluación multidisciplinar, compuesto por profesionales del Hospital, de atención primaria, de salud responde, 061 y de Linde HealtCare. Los resultados obtenidos han sido muy positivo, consiguiendose entre todos, que los pacientes controlen gran parte de estos episodios de empeoramiento, evitando tener que acudir a urgencias o ingresar.
